Virtual Ordering Options for Direct to Patient Care
To better serve you and your practice, Myriad is offering two different processes to ship the Myriad myRisk® and BRACAnalysis® test kits directly to your patients’ homes and support the patient with any sample collection questions that may arise.

Login to our online portal, MyriadPro, to submit the Myriad myRisk® test request form electronically and indicate you want the kit sent directly to the patient.
Once that has been done, the following fields will be required to proceed:

- Login to our secure online portal, MyriadPro.
- If you are already a MyriadPro user, login to your account here.
- To register for MyriadPro, click here. For instructions on how to register and use MyriadPro – click here.
- Fill out an electronic test request form (eTRF) for Myriad myRisk®, and be sure to indicate that you want the test kit delivered to the patient for sample collection by checking the box.
- Login to our secure online portal, MyriadPro.

- Patient name and Address, City, Zip Code, State
- Your e-signature for authorization – check the signature box

- Once you ‘submit’ the test request form, the Myriad team will send this completed TRF along with a saliva Myriad myRisk test kit within 48 hours directly to the patient’s home. The kit will include specimen collection instructions, a pre-paid return shipping label, and a return envelope.
- If a patient wants to learn more about genetic testing, they can call Myriad’s pre-test educational phone line to speak with a certified genetic counselor. This phone number is included in the test kit.
- The patient will then need to complete the specimen collection and mail it back to Myriad with the signed TRF AND a front and back copy of their insurance card inside the kit for their sample to be processed.
- Once Myriad has received the patient’s sample – turnaround time, billing, and report delivery will proceed as normal.
- For those that are new MyriadPro users – your results will also be delivered through MyriadPro and you will receive an email when results are posted.
- After you share results with your patient, they have the option to schedule an educational appointment with a certified genetic counselor at www.my.myriad.com/consults. You will receive a copy of the call notes for your records should the patient pursue this.

- Use this cover sheet to indicate which test kit you are ordering and that you want the test kit sent directly to the patient’s home.
- Email or fax the TRF with cover sheet to Myriad
- Email the TRF to csaccessteam@myriad.com
- Fax the TRF to 801-584-3615
- Myriad will review and then mail your completed TRF along with a test kit directly to the patient. If a patient wants to learn more about myRisk genetic testing, they can call Myriad’s pre-test educational phone line to speak with a certified genetic counselor. *IMPORTANT INSURANCE INFORMATION: Myriad will need a front and back copy of your patients’ health insurance information. There are two ways this can be provided:
- Provider sends copy with email/fax
- Patient provides copy in test kit when mailing back to Myriad
